FORMÉ TERMS OF SERVICE

These Terms of Service ("Terms") govern your use of the FORMé website located at onforme.co and your purchase of FORMé products. By accessing the site or placing an order, you agree to these Terms.

1. About FORMé

FORMé is a Canadian supplement brand selling FORMé Daily, a daily immune supplement powder. FORMé is not a pharmaceutical company. Our products are natural health products and are not intended to diagnose, treat, cure, or prevent any disease.

2. Eligibility

You must be 18 years of age or older to purchase from onforme.co. By placing an order you confirm that you meet this requirement.

3. Products and Health Disclaimers

FORMé Daily is a natural health product. Results vary by individual. The statements made on this website have not been evaluated by Health Canada or the U.S. Food and Drug Administration. FORMé Daily is not intended to diagnose, treat, cure, or prevent any disease.

Consult your healthcare provider before using FORMé Daily if you are pregnant, breastfeeding, taking prescription medications, or have an underlying health condition. FORMé Daily is formulated for adults and is not recommended for children.

4. Orders and Pricing

Prices are displayed in the local currency of the customer where supported, or in Canadian dollars (CAD) by default. The currency and final amount will be confirmed at checkout before payment is processed.

We reserve the right to modify prices at any time. Prices at the time of your order will be honoured for that order. We reserve the right to refuse or cancel any order at our discretion, including orders that appear fraudulent or where a pricing error has occurred.

5. Subscriptions

FORMé Daily is available on a subscribe and save basis. By subscribing you authorize us to charge your payment method on a recurring 30-day basis at the then-current subscription price.

You may pause or cancel your subscription at any time from your account page with no penalty. Cancellation takes effect immediately for future billing cycles. Orders that have already been processed and shipped are not eligible for cancellation refunds, except as covered under our 365-day guarantee.

We reserve the right to modify subscription pricing with reasonable advance notice to subscribers.

6. Shipping

We ship within Canada and the United States. Shipping is free on all orders. Estimated delivery times are 5–10 business days within Canada and 7–14 business days to the United States. We are not responsible for delays caused by carriers or customs authorities. International duties, taxes, or import fees applicable in the destination country are the responsibility of the customer.

7. Intellectual Property

All content on onforme.co including text, images, logos, and brand assets is the property of FORMé and may not be reproduced, distributed, or used without written permission.

8. Limitation of Liability

To the maximum extent permitted by applicable law, FORMé shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our products or website. Our total liability shall not exceed the amount paid for the order giving rise to the claim.

Nothing in these Terms limits or excludes any rights you may have under applicable consumer protection legislation that cannot be waived by contract, including rights available to consumers under the laws of your province, state, or territory of residence.

9. Governing Law and Dispute Resolution

These Terms are governed by the laws of the Province of Alberta and the federal laws of Canada applicable therein.

For customers located in the United States: you agree that any dispute arising from these Terms or your purchase will be resolved through binding individual arbitration administered under the rules of a mutually agreed arbitration body, and not through class action. Nothing in this clause limits rights you may have under applicable US federal or state consumer protection laws that cannot be contractually waived. Customers in California retain all rights available under the California Consumer Privacy Act and California consumer protection statutes.

10. Changes to These Terms

We may update these Terms from time to time. Continued use of the website after changes constitutes acceptance of the updated Terms. The date at the top of this page reflects the most recent update.
